For my last patient of the day in what could potentially be my last clinic in surgery, ever, I had someone whom I couldn't help. It is never a good feeling to not be able to help your patients. It was an elderly lady who came with an incurable cancer of the biliary tract. She was in lots of pain and the irony was that she didn't know about her ailment. To her this was just another abdominal pain that might go away with medication. Her pain was intractable and wouldn't abate even with morphine and my pain is here to stay. Nevertheless I have come to realize that we as doctors have our limits and these limits we can't exceed no matter how hard we try. Somewhere something gets out of control and then nothing can be done. So long for the team AKU.
